How to file a health insurance claim with United India Insurance?

2 types of health insurance claims can be filed with United India Insurance:

Cashless Claim: Get admitted at a network hospital of the insurer to avail cashless hospitalisation. Intimate the insurer prior to admission in the case of planned hospitalisation or within 24 hours of admission in the case of emergency hospitalisation. Show your health card at the hospital helpdesk. The hospital will submit a pre-authorisation request form to the insurer or Third-Party Administrator. Upon approval, the hospital will provide cashless treatment to the insured member. The medical bills will be settled directly with the hospital by the insurer. Reimbursement Claim: A written intimation has to be sent to the insurer or TPA within 24 hours of hospitalisation. Collect all the original medical documents at the time of discharge. Pay the bills upfront and later file a reimbursement claim with the insurer within 7 days from the date of discharge. The claim will be approved or rejected as per the policy terms and conditions.

Process Dos Don’ts
New policy
  • The proposal form submitted by the proposer should contain his or her personal details including contact number and email ID.
  • Read the prospectus carefully before filling out the proposal form. It contains details of the chosen health insurance policy.
  • The insured person details form has to be filled with details of each family member that is to be covered under the policy. Along with the form, a stamp-size photograph and an ID of each family member like Voter’s ID, Passport, PAN card or Driving Licence has to be submitted to the insurer.
  • At the time of proposal, honestly disclose any pre-existing illness, injury or ailment for each insured person.
  • For any clarification regarding the proposal form, contact the insurer’s office.
  • A copy of the duly-filled and signed proposal form will be given to the proposer along with the policy copy within 30 days of policy issuance.
  • Check the name, age, gender, address, and other personal details in the policy schedule. Ensure the period of insurance is correct.
  • Don’t furnish any false information on the proposal form.
  • Don’t sign the proposal form without reading the terms and conditions.
  • Don’t hide any facts or material information like pre-existing conditions of the insured member.
Renewal
  • Keep track of the policy renewal date and renew the policy before the due date to enjoy continued benefits.
  • Pay the renewal premium within the 7-days grace period to avoid policy lapse.
  • Read the sum insured enhancement condition carefully before requesting for enhancement of sum insured.
  • Changes at the time of policy renewal have to be given in the form of written requests.
  • A receipt of the premium payment has to be kept under Indian Law
Don’t delay in policy renewal even by a day or else claim will be not be payable.
Cashless claim
  • Ensure the hospital falls under the network hospitals of the insurer to avail cashless facility.
  • In the case of a planned hospitalisation, the TPA has to be given prior intimation with the name and address of the hospital.
  • In the case of an emergency hospitalisation, the TPA has to be intimated immediately after admission.
  • The hospital has to send a duly-signed pre-authorisation request by the insured to the TPA. Admission details, illness, treatment, and expense have to be mentioned in the form.
  • Upon discharge from the hospital, pay the difference amount.
  • Pre and post-hospitalisation expenses can be claimed individually. In the case of post-hospitalisation claim, all original documents have to be submitted within 7 days to the TPA.
Don’t delay the submission of the documents required by the TPA or the insurer.
Reimbursement claim
  • In the case of reimbursement claims, the TPA has to be intimated within 24 hours of hospitalisation.
  • Obtain the discharge summary, investigation report, hospital bills, pharmacy bills, and prescriptions from the hospital at the time of discharge. The original documents have to be submitted to the TPA within 7 days of the discharge date.
  • Separate claims can be made for pre and post-hospitalisation expenses. All original documents have to be submitted within 7 days to the TPA for a post-hospitalisation claim.
  • Keep a copy of all the medical documents.
  • Don’t forget to disclose insurance details at the time of admission.
  • Don’t forget to read the pre-authorisation request form carefully before signing it.
  • Don’t hide facts to avoid facing a dispute at the time of making a claim.
Grievance
  • Write a mail to the policy issuing office regarding any complaint or grievance the policyholder may have with the services rendered by the insurer.
  • Approach the Regional Office or Head Office for redressal.
  • If the loss is below Rs.20 lakh and the policy is issued to an individual, the grievance can be escalated to the Honorable Ombudsman (if the policyholder is dissatisfied with the redressal carried out by the insurance company).
